For some people, chronic pain can be influenced by an overprotective nervous system that continues to produce pain even when the body is no longer in danger. The encouraging news is that these pain pathways can change. By understanding pain differently and gradually helping the brain and nervous system learn safety, recovery may be possible.

What Is Pain Reprocessing Therapy?

Pain Reprocessing Therapy (PRT) is an evidence-based psychological approach designed to help people with primary chronic pain — pain that is being generated or maintained by the brain and nervous system rather than ongoing tissue damage.

PRT uses education and practical techniques to help you interpret sensations with less fear and gradually change the brain’s learned response to them.

The aim isn’t to ignore or push through pain, but to help your nervous system recognise that previously threatening sensations can be safe.

Why Can Chronic Pain Persist?

Chronic pain can develop when the brain and nervous system become increasingly protective and begin interpreting sensations as threatening, even when there is no ongoing tissue damage requiring protection.

This can be influenced by factors such as previous injury, stress, fear of pain, learned associations, attention to symptoms and difficult life experiences.

Over time, these patterns can become learned — but because the nervous system is adaptable, they can also change.

How Pain Reprocessing Therapy Works

PRT helps you change the way you respond to pain and other physical sensations. Through education, guided exercises and techniques such as somatic tracking, we work on interpreting sensations through a lens of safety rather than danger.

As fear around symptoms reduces and the brain receives repeated messages of safety, learned pain pathways may begin to change. The process is gradual and personalised, helping you build confidence in your body and return to activities that may have become associated with pain.

Can Chronic Pain Improve?

Yes. Research shows that some forms of persistent pain can significantly improve — and for some people resolve — when the brain and nervous system learn to interpret previously threatening sensations as safe.

Recovery is individual, and no outcome can be guaranteed. The aim is to reduce fear, build confidence in your body and help you gradually return to a fuller, more active life.

Could Pain Reprocessing Therapy Be Right for You?

PRT may be appropriate for some persistent pain presentations where ongoing tissue damage does not fully explain the symptoms, including:

  • Persistent back or neck pain
  • Fibromyalgia
  • Chronic headaches or migraines
  • Repetitive strain symptoms
  • Some forms of pelvic pain
  • Other persistent or recurring pain without a clear ongoing structural cause

A careful assessment is important to determine whether this approach is appropriate for you.

Please ensure that you have consulted with a medical doctor who thoroughly investigates and rules out potential causes of pain, including:

  • neurological impairment
  • cancer tumour
  • autoimmune disease such as type 1 diabetes, multiple sclerosis, lupus, and rheumatoid arthritis
  • bone fracture
  • infection

Chronic headaches & migraines, chronic jaw and fascial pain, chronic pelvic and bladder pain, back, neck or joint pain, irritable bowel syndrome (IBS), fibromyalgia, chronic fatigue syndrome (CFS/ME) are just some of the common causes of chronic pain affecting a significant portion of the UK population.

The research behind PRT

PRT blends pain education, cognitive behavioural therapy, mindfulness practices, and gradual exposure to rewire your brain's response to pain. PRT tackles neuroplastic pain, also referred to as nociplastic pain, with science-backed methods enabling you to reappraise the source of your pain and its threat level.

Neuroplastic pain is a type of pain that persists beyond the expected healing time for an injury or illness, or pain that has originated from an unknown source. Unlike acute pain, which serves as a warning signal for tissue damage or injury, neuroplastic (nociplastic) pain is characterised by changes in the nervous system that result in the amplification and continuation of pain signals.

PRT is effective in providing relief for individuals experiencing chronic back pain. A randomized controlled study published in JAMA Psychiatry (2021) compared PRT and placebo. The results revealed that 66% of participants after engaging in the four-week PRT psychological treatment, were pain-free or nearly pain-free post-treatment. Importantly, the majority of these individuals sustained this relief for up to one year. In 2023 research confirmed the link between the reduction in pain and the changing belief that pain is caused by the brain rather than a structural problem with the body. This change occurred due to the clients practicing pain reprocessing therapy!

Why This Work Matters to Me

I understand persistent pain from both a professional and personal perspective. I lived with chronic low back pain for around two years and was offered spinal fusion surgery, but chose not to proceed.

Learning about pain neuroscience and neuroplastic pain changed the way I understood my own symptoms and played an important part in my recovery.

That experience ultimately led me to train in Pain Reprocessing Therapy and now informs the compassionate, evidence-informed way I support others living with persistent pain.
